I just got rejected by iTunes store with the following reason:
Guideline 4.0 - Design
We noticed an issue in your app that contributes to a lower quality user experience than Apple users expect:
- Your app has poor UI experience and usability
Next Steps
To resolve this issue, please revise your app to address all instances of this type of issue.
Now with this Plethora of information, what am I suppose to do?

have you tested your app on a device? Is it functioning as it should? Maybe certain buttons don't work?
If this is all OK, then maybe show a screenshot of your app. Apple rejects apps which are too limited/simple and don't offer the user much value.
They lately also reject apps which are an obvious repackaged template.
This straight from the guidelines:

Maybe share a screenshot of the app? Might be helpful if you want better feedback.
